ID teams are groups of individuals who are collectively responsible for treatment/training decisions for a particular client, and whose composition derives from the identified needs of the involved client.
Interdisciplinary Team Process
The composition of the ID team includes the client, social worker, team leader, aides representative of shifts, the client's authorized representative or guardian, and any educational, vocational, nursing, medical and clinical staff having active involvement with the client. Social wokers, as QMRP's, chair the interdisciplinary teams.
ID Team Functions
- Identification and assessment of a client's treatment/training needs.
- Development of an individual program plan designed to meet client needs.
- Revising or modifying the program plan as necessary and appropriate.
- Monitoring the implementation of the individual program plan.
- Reviewing at least annually the client's progress toward stated goals and objectives.
Each client's plan must provide active treatment, but it should also be person-centered, consistent with his or her values and beliefs.
